I lost my (wallet / purse) which contained several forms of identification, checks, credit cards, etc. Someone may try to use my name, address, social security number, and other personal information to fraudulently obtain credit and to purchase services and merchandise.
Please place a "Fraud Alert" on my credit file. I request the alert state that "NO CREDIT IS TO BE ISSUED IN THE NAME OF (insert your name) WITHOUT AUTHORIZATION BY (insert your name) AND SUPPORTING IDENTIFICATION. This alert should remain on my file indefinitely.
